Join us and shape the future of healthcare!POSITION PURPOSEThe primary purpose of your job position is to assist with transporting PACE/LIFE participants to and from the PACE/LIFE Center and to other appointments. To promote safety and comfort of participants while waiting for transport or appointments. To communicate with drivers and the center, providing relevant information regarding status of appointments, participants and any problems or issues.ESSENTIAL FUNCTIONSKnow, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Trinity Health and Trinity Health PACE (TH PACE) in behaviors, practices, and decisions.Consistently demonstrate (leads by example) TH PACE Values to all internal and external customers(Participants, visitors, volunteers, and colleagues.) Create and maintain an atmosphere of warmth, propagating a calm environment throughout the office.Execute assignments in a culture that is shared and collaborative across all divisions within TH PACE.Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. Follow work assignments, and/or work schedules in completing and performing your assigned tasks.Promote and maintain collaborative relationships with managers, peers, and customers by effectively fostering a team environment, building consensus and resolving conflicts.Maintains a working knowledge of applicable Federal, State, and local laws and regulations, TH PACE CorporateIntegrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.In light of a disaster will know your disaster recovery, crisis management and business continuity plans and act within your role that is developed within the business continuity plan. Which may include working at another location, remotely from home, and maintaining constant contact with key personnel.Attend and participate in scheduled training, in-service training, mandatory annual in-service training and educational classes as required/needed. Assist driver in transporting participants in a safe manner in compliance with the PACE/LIFE Organization operating state licensing regulations.Assist with provision of safe, comfortable transportation, accompanying driver on the bus, calling participants from the bus and communicating with the center as needed. Radio the Transportation Supervisor/dispatch at PACE/LIFE after every pickup and drop off.Accompany selected participants to medical and other appointments, assisting with ambulation and toileting as needed and providing supervision to assure participant’s safety and comfort.Provide receptionist with information needed to make contacts with medical offices, participants, family members, and others regarding transport scheduling.Assist drivers in daily vehicle cleaning and the proper sanitizing of the vehicle.Regularly communicate information as a part of the interdisciplinary team. Report any concerns regarding participants to the Transportation Department as soon as possible.If applicable, provide information to the receptionist or designee by time outlined by PACE/LIFE daily regarding any participants who will not be attending the center and the reason.Transport/assist with transport of supplies, specimens, pharmaceuticals, meals, and mail.Provide support to participants on recreation activity trips and assistance with toileting on outside trips when needed.MINIMUM QUALIFICATIONSMust possess a comprehensive knowledge obtained through a high school diploma or equivalent. Transportationand human services experience preferred.
