Position acts in the role of a mentor for new staff to case management.Position is the subject matter expert for staff questions regarding process.Other duties pertinent to the day to day operations of the department may be assigned as needed.ESSENTIAL DUTIES AND RESPONSIBILITIESInclude the following. Others may be assigned. Carries out the Case Management process with members including member assessment, planning, facilitation, care coordination, evaluation and advocacy for options and to meet the member’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes. Exceeds established CMDM productivity standards. Assists in training/mentoring of new Case Management staff. Under the direction of the Supervisor/Manager, performs monthly 1:1 with staff, offers input to manager on30 -90 day evaluations, midyear and annual performance evaluations Assists in designing and implementing orientation, ongoing training and new program implementation Supports a positive workplace culture by adhering to Conifer workplace guidelines and maintains and promotes confidentiality of patient information and adheres to HIPAA regulations Regularly participates in Medical Services and CMDM Team meetings and work groups Provides overtime coverage for Case Management staff when necessary Other duties as assigned.Qualifications: KNOWLEDGE, SKILLS, ABILITIESTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
