Every other Weekend, Saturday & Sunday 8 a.m. to 8 p.m.Job Summary:In collaboration with the entire interdisciplinary team, utilizes clinical judgement and expertise to coordinate care management interventions. Develops strategies to ensure cost effective, quality care outcomes to meet a patient/family’s end-of-life care needs. Assess, plans, monitors and provides relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.Spectrum Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. They also experience healthier work-life balance with the flexibility that comes with hospice care for patients in their homes.Essential Functions:* Works in close collaboration and communication with physicians and other members or the healthcare team to minimize duplication of effort, promote teamwork and ensure seamless coordination of services within and between teams and across campuses.* Communicates with other members of the health care team in order to ensure continuity of care and coordination of services.
