The RN Case Manager is responsible for utilization review, discharge planning, overall clinical management, and resource management of patients in the acute care setting.# Based on patient assessment, the RN Case Manager ensures provision of care in the most appropriate setting utilizing indicated resources.# The RN Case Manager ensures a multidisciplinary, collaborative team approach to managing all patients in the most efficient and effective manner; proactively plans alternatives to acute hospitalization in collaboration with the multidisciplinary team, including the physician and determines available community resources to facilitate timely discharge of patients to the most appropriate post-hospital setting. Education: RN graduate of an School of Professional Nursing Experience: Previous experience preferred.## Licenses/Certificates:# License to practice in the South Carolina or Compact Licensure Agreement
