Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.
Education – Bachelor’s Degree in Nursing
Experience – 5 years of experience typically gained through skills/knowledge/abilities in previous nursing or CDE clinical experience
Registered Nurse (RN)* Valid State of Michigan RN license
Basic Life Support (BLS) Note used for BLS,CPR & BCLS) Medical Group, Grand Rapids Hospitals, Spectrum Health System 90 Days
