Description
Initial areas of focus are:
1) Assist the PM of Outreach & Diversion to implement a plan in coordination with the City of Atlanta and other community stakeholders to decommission encampments and move households experiencing unsheltered homelessness into housing; and
2) Lead the monitoring and reporting on the effectiveness of decommissioning efforts, making recommendations for future improvements.
- Assist the PM of Outreach & Diversion with coordinating with outreach teams to ensure individuals have access to immediate shelter, healthcare, and social services during the decommissioning process (when applicable).
- Conduct site assessments to evaluate the conditions of encampments and identify resources needed for safe decommissioning.
- Monitor and report coordinated entry assessments and track document readiness based BNLs of encampments.
Role and Responsibilities
- Identify all encampment areas in the Atlanta geographic region and assess capacity needs to provide outreach and decommissioning efforts- in close partnership with PM of Homeless Services to identify priorities
- Plan, coordinate and schedule all encampment decommissioning operations
- Assist with overseeing operations and oversee work performed by all entities
- Identify and assess safety concerns and engage with law enforcement and public health representatives as appropriate
- Implement strategic plan and efforts for all individuals to move from encampment to housing or shelter before closure
- Identify strategy for transportation and storage of personal valuables
- Facilitate weekly case conference with providers
- Work with crisis response and outreach teams to build By Name Lists (BNL) to ensure that those in encampments and unsheltered are accurately reflected in Coordinated Entry.
- Identify and advocate for solutions needed on an individual level for those experiencing unsheltered homelessness.
- Monitor the effectiveness of decommissioning work to ensure those in interim housing are gathering documents needed for permanent housing.
- Ensure regular reporting of encampment monitoring and pre-post cleaning/mitigation efforts for monthly summary.
- Serve as a lead project manager for decommissioning, monitoring and repopulation prevention for planned encampment closures.

Operational Duties:
- Co-lead closure teams meetings to ensure coordination and collaboration with Project Manager of Outreach and Diversion.
- Facilitate check ins with diversion teams to ensure goals are being met.
- Monitor diversion project data to identify gaps and propose solutions.
- Visualize data to report on encampment outcomes & repopulation activities
- Operationalize best practices with encampment closures.
- Prepare weekly report outs on encampment population numbers and progress toward housing goals.
- Utilize the Homeless Management Information System (HMIS) to monitor CE Assessments and document readiness for encampment BNLs.
- Work closely with Coordinated Entry Project Manager to ensure appropriate documentation provided, confirm unit availability as needed and identify bottlenecks to obtaining housing.
- Assist with identifying prioritized individuals based on CoC prioritization policies.
- Provide exceptional customer service to our Continuum Partners, partnering with them to ensure a transparent system is in place.
- Ensure cross disciplinary teamwork with other verticals.
- Participate and provide coverage and support to other vertical lines as needed.
- Represents Partners for HOME in community meetings, committees and work groups as needed.
Qualifications and Education Requirements:
- Individuals who have lived experience with homelessness strongly encouraged to apply
- Spanish Bilingual proficiency preferred but not required.
- High energy and a passion for the CoC’s mission and goals
- Previous experience working with the homeless population or housing preferable.
- Skilled in Excel and have experience working in HMIS.
- Proficiency in Power BI or other data management tools is a plus.
- Familiar with housing subsidies such as: Emergency Housing Voucher, Georgia Housing Voucher, Permanent Supportive Housing
- Strong initiative and curiosity.

Salary: $52,800 – $66,000
